窦口鼻道复合体CT影像解剖与慢性鼻窦炎的相关性研究

摘要:
目的:研究窦口鼻道复合体(ostiomeatal complex,OMC)的CT影像解剖与慢性鼻窦炎的相关性,为OMC的研究提供影像解剖学的新思路。方法:对67例头颅冠状CT薄层扫描片进行观测,包括:中鼻道宽度,筛漏斗长度、宽度,钩突厚度、内倾角度,筛泡、鼻丘的内径,并对测量结果与鼻窦炎积分进行相关性分析。结果:67例(134侧)中钩突头端的附着部位有3种类型:筛顶型(23.13%)、纸样板型(56.72%)和中鼻甲型(20.15%),这3种类型的比例在鼻窦炎组与非鼻窦炎组有较大差异(P<0.01)。鼻窦炎组与非鼻窦炎组筛漏斗长度有显著差异(P<0.01)。筛漏斗长度及钩突角度与鼻窦炎积分之间有显著相关性。结论:OMC解剖因素(包括筛漏斗及钩突的发育等)与慢性鼻窦炎发病有密切关系。

Abstract:
Objective:To study the correlation between imaging anatomy of the ostiomeatal complex (OMC) and chronic sinusitis, and provide a new way of anatomy and radiology for OMC study. Methods: Sixty-seven coronal CT slices were observed. The items observed including: attachment of the superior part of the uncinate process, highness and thickness of the uncinate process, length and width of the ethmoidal infundibulum, diameter of ethmoid bulla and agger nasi. The correlation between the results of measurement and the scores of the sinusitis patients were also analysed. Results: The superior part of the uncinate process may attach to 3 different structures:the skull base, the lamina papyracea and the middle turbinate, their proportion in sinusitis patients and non-sinusitis persons showed notable divergence. The ethmoidal infundibulum of sinusitis patients was obviously longer than that of non-sinusitis persons (P<0.01). The length of infundibulum and the angle of uncinate process were correlated to the score of the sinus opacity of sinusitis patients. Conclusion:There is an intimate relationship between the anatomic factors of OMC and the initiation, development of chronic sinusitis.
